sometimes aged care the pressure of two years of a pandemic followed by opening up despite the emergence of omicron which wasn t factored in to those plans means nurses are struggling wherever they work midwives are coping with a baby boom that hasn t been accompanied by increased resources allied health staff have been pulled from their areas of specialty to provide fundamental nursing care assistance registered undergraduate students of nursing or midwifery rusons and rusoms are working alongside nurses and midwives in a team model that was never part of their role and doctors and working longer hours than their usually unsafe exhausting schedule as one of my medical colleagues wrote after reading the first version of this post on fb and add to it all that in the current wave numerous staff are testing positive with community acquired covid thanks to the nature of omicron most are only very mildly unwell as we are all vaccinated but the health service i work at had over 7 of all the work force off with covid according to an all staff email last week and at least as many more off because a member of their household had tested positive it simply isn t possible to do business as usual with that many staff unavailable and the numbers testing positive and the numbers of covid patients adding to usual work loads rise every day the system is under strain like never before and the numbers are increasing some people have snidely asked me on twitter how much the vaccine mandate has had to do with staff shortages i don t have official figures but i ve heard informed estimates of 150 175 victorian nurses and midwives from a workforce of some 90 000 it s not that it s the impact of unrelenting numbers stretching a system that in every state and territory was running at or near 100 capacity before covid existed when you view healthcare through a financial lens rather than as an essential service decisions focus on cost cutting and the highest cost is staffing the primary reason for in patient care is for nursing from monitoring to interventions and nurses are the biggest part of the workforce hospital staff are paid fortnightly so pay roll can process our pay one week and everyone else s pay is processed in a separate cycle as hospitals seek to reduce the nursing care we provide through measures like hospital in the home patient churn has dramatically increased churn is the turnover of hospital beds nobody gets one unless their care can t be delivered otherwise so someone with sub acute bacterial endocarditis a bacterial infection that irritates the lining of the heart now has an rn visit three times a day for iv antibiotics instead of having a six week inpatient stay and while this has all kinds of patient benefits it s less disruptive they have far fewer complications that i fully support it also means in patients are more acutely ill they have more comorbidities need more interventions and are at higher risk of suddenly deteriorating all of which means they need more nursing care add covid to that mix and my clinical colleagues wherever they re working in whatever role are weary to the bone the problem with let it to minimise covid transmission by modifying our behaviour through public health measures including social and physical distancing masks episodic lockdowns check ins contact tracing and immunisation the australian government has adopted an individualistic personal responsibility approach it frames as living with the virus this is colloquially referred to as letting covid rip maybe let it rip would work with something like rubella if you were a monster sure any pregnant woman contracting it would have an increased risk miscarriage stillbirth and of her baby developing any combination of congenital cardiohepatosplenic disease impaired vision and or hearing and intellectual disability but that s only if she contracts it in the first trimester and not even a problem for all first trimester foetuses and sure it can cause encephalitis in children and adults but that s rare joint pain that takes a month or more to settle is more common but only in women which means only 35 of adults would be temporarily affected by it and the disease itself is almost always mild but even if you agree that these risks are acceptable on a macro level you only get rubella once and the vaccine is almost 100 effective for life for almost everyone let it rip is not a viable aim for a disease you can contract more than once without guarantee that subsequent episodes will be decreasingly severe and with each bout carrying the possibility of developing any one of dozens of long term disabilities that can affect almost any system covid is an airborne virus that most likely affects small vessels resulting in tiny clots that s a major simplification of course it can also involve hyperinflammatory responses widespread coagulopathy blood vessel damage changes to blood cell morphology and survivors can be left with damage from infertility and erectile dysfunction to irreversible impairment of any vital organ extreme fatigue to neurological impairment equivalent to a decade of ageing there are international reports of people contracting covid more than three times so far with differing symptoms and post viral effects from nothing to profound debility or disability each time odds are a virulent genuinely mild variant will evolve eventually and become the dominant strain that this will become something we can vaccinate against annually and only worry about if we re older or immune compromised which is absolutely not me saying either of these groups is okay to be put at risk but omicron is not that variant and we are a long way from being able to live alongside this virus
