lotus surviving life remaining pure to urself lotus surviving life remaining pure to urself monday may 09 2011 just a thought i am just too lazy to blog these days and given the confidential nature of my work i have to be careful what i write here today a patient asks me to be his hiv physicians during my rotation at my current hospital have to say i kinda feel honoured being asked to do so especially me being so early on in my specialty i am still a baby hiv physician trainee oh well i will start to have my own immunocompromised patient list by end of this month so many new things to learn the new anti retrovirals drugs combo just keep growing and with a new horizon of treatment for hepatitis c i think i will struggle just to keep up and doing the infectious disease on call is still pretty exciting for me i start to get the hang of the usual malaria tb stuffs meningitis and encephalitis stuffs fever of unknown origin but there are still rare ones like chikungunya i was like chicken what not a very exciting disease actually i still go wow wouldn t say i missed acute medicine on call but i do feel slightly less confident in managing general stuffs i am still quite amazed to get call as far out as bristol birmingham since i thought they will ring london school of hygiene and tropical medicine probably its just me who find all this stuffs very exciting its just soo 3 diplomas i need to do as part of my exit exam bring it on i am moving on to a greener grass of doing non acute non resident 24 hour on calls bloody yeay lets hope it will match my expectations and i will be learning a hole lot of new stuffs exciting probably fancy doing lab tests i used to enjoy doing this acute on calls but i start to find some house officers incredibly annoying no sense of trying to learn or working out what s wrong with the patients doing tests for the sake of tests patients suffered post op complications as simple as urinary retention after surgical discharge cos nobody bothered to examine someone s tummy i will go mad if i was the patient don t even start on people leaving can do in the morning clinical procedures for night team gosh set my temper flying been a rather odd week medically speaking i had a few young men who all ended up with diagnosis of vasculitis and all of them have visited egypt few weeks either during or prior evolvement of the disease one of them is very sick indeed having vasculitis affecting his kidney his gi tract bleeding pr skin developing pressure sore which is odd in an active and prior to this perfectly healthy 20 sth man probably infarcted his tonsillar artery and it is just awful the renal team is hitting him hard with cyclophosphamide which cause azoospermia and probably have to think of other immunosuppressive drugs if this doesn t control his disease the other one is looking more like limited wegener s and we are pulsing him with methylprednisolone again coming back from egypt and in clinic i had someone who developed reactive arthritis post diarrhoea in egypt and the clinical picture is looking more and more like ankylosing spondylitis odd indeed posted by lotus at 2 08 pm 0
